Title: Comparative Study between Low Level Laser Therapy and Intra Tympanic Steroids Injection in Treatment of Tinnitus
Background: Persistent tinnitus can result in serious problems and morbidity, especially at the psychological and socio-professional levels. Although many treatments exist for the resolution of tinnitus symptomatology, total eradication of symptom rarely occurs. The aim of the work: To compare the effectiveness of two new promising techniques in treating tinnitus; low level laser therapy versus intra tympanic dexamethasone injection.
Patients and Methods: A comparative randomized clinical trial included 40 patients suffering from tinnitus for more than 6 months. They were divided into two groups randomly with every other patient consecutively; group [1] included 20 patients received steroids injection [0.5 ml of 4 mg\ml dexamethasone] on six sessions twice weekly, and group [2] included 20 patients who obtained low level laser therapy transmeataly with wave length 650 nm and laser output 5 mW for 7 sessions. Assessment of treatment included tinnitus handicap inventory [THI], visual analogue scale of loudness [VAS-L], pure tone audiometry and tympanometry for evaluation before and after therapy.
Results: THI after treatment was lowered in group 2 more than group 1, but this result did not reach statistical significance [P=0.076]. Quality of life in group 2 after treatment was statistically significantly better than in group 1 [better results after treatment as regard questions assessing sleeping at night, interfering with daily work and concentration].
Conclusion: Low level laser therapy is more effective and safer than intratympanic steroid injection in treating chronic tinnitus with duration more than 6 months. Title: : Radiographic Evaluation of Position and Angulation of Impacted Maxillary Canines in Cleft Palate Cases
Objective: To evaluate the position and angulation of impacted maxillary canines in unilateral and bilateral cleft lip and palate cases and compare them to noncleft cases.
Material and method: In this study 60 patients OPGs were taken out of which 20 (12 males, 8 females) presented with unilateral cleft lip and palate (UCLP) (Group I), 20 (15 males, 5 females) had bilateral cleft lip and palate (BCLP) (Group II) and 20 (13 males, 7 females) had no clefts (Group III). All the subjects had impacted maxillary canines (bilateral or unilateral). Cases with 2/3rd canine root completion were selected. Intruis suite R2 software was used to determine the position and angulation of the canine.
Results: A total of 63% (37) of the canines were in Sector 1, and 31% (18) were in Sector 2. In-depth of canine impaction relative to the root of the incisor maximum canines were in Grade II and Grade III at 45% (27) and 36% (22) respectively. Angulation of the canine relative to the intercondylar line was seen most in Group II (75%) and (19%) in Group I as compared to the noncleft group (Group III). No difference was seen in the position of canine impaction for buccally and palatally canines and for the unilateral and bilateral cleft groups or the noncleft and cleft groups.
Conclusions: The majority of the impacted canines had cusp tips located in sectors II and III. In almost all cases, angulation did not increase the prediction of eventual impaction, although it may have contributed slightly in Sector II.

Title: Prevention and Improved Treatment of Diabetes using a Micronutrient Mixture, Probiotics, Omega3, and Collagen Peptides
Diabetes mellitus is a progressive chronic disease associated with enhanced levels of fasting glucose, HbA1C, and insulin resistance. Despite diabetic medications, which reduce the blood level of glucose but not the causes of the disease, complications such as retinopathy, nephropathy, peripheral neuropathy, and heart disease develop. External stressors which include poor diet and lifestyle, and excessive exposure to environmental toxins increase the levels of certain internal stressors such as oxidative stress and chronic inflammation. These internal stressors damage the insulin receptors causing accumulation of glucose in the blood leading to diabetes type II. Despite valuable recommendations for improving diet and lifestyle, and avoiding environmental toxins, the risk of developing diabetes type II continues to increase. In addition to elevated levels of oxidative stress and chronic inflammation, there are other internal stressors which include intestinal dysbiosis, omega-3-fatty acids dysfunction, oxidative damage to insulin receptors, and increased activity of dipeptidyl peptidase-4 (DPP-4), which prevents the secretion of insulin from the pancreas to the blood, contribute to the development and progression of diabetes type II. Addressing one of these internal stressors at a time may not produce an optimal benefit. To prevent and improve treatment of diabetes type II, this review proposes to address all external and internal stressors at the same. The proposal includes (a) adoption of good diet, healthy lifestyle, and reducing exposure to environmental toxins, (b) supplementation with a micronutrient mixture which would simultaneously reduce oxidative stress and chronic inflammation by elevating the levels of antioxidant enzymes and antioxidant compounds, (c) probiotics with prebiotics which would reverse the intestinal dysbiosis, (d) in patients with insulin resistance, omega-3-fatty acids directly activate the insulin receptor-linked signaling protein AKT which signals glucose transporter-4 (GLUT-4) to migrate to the cell surface and allows entry of glucose inside the cells, and (e) collagen peptides which increase the secretion of insulin by inhibiting dipeptidyl peptidase-4 (DPP-4) activity. The proposed plan may prevent and in combination with drugs may improve their effectiveness in diabetes type II.

Title: Effects of Pain Neuroscience Education and Physiotherapy on Chronic Low Back Pain, Fear of Movement and Functional Status: A Randomised Pilot Study
Background: Chronic non-specific low back pain is a non-harmous condition often found in the general population. It is one of the most significant disabilities and needs different treatment modalities. This study investigates the effects of pain neuroscience education and physiotherapy on pain intensity, fear of movement and functional status in a Lithuanian cohort with non-specific low back pain. Methods: The study was performed at the primary health care unit in Kaunas, Lithuania. The key inclusion criterion was persistent non-specific low back pain longer than three months and which affects daily life functions. Thirty participants (mean 33.47, SD 4.38 years age, 70% women) were randomised into two training groups with and without pain neuroscience education (for a total of 60 min of teaching). Physiotherapy was performed twice per week during 45 min/session for a period of 10 weeks with exercises which strengthen, stabilize, and stretch the spinal cord muscles. Outcomes included pain intensity, kinesiophobia and disability and these were measured by self-scored questionnaires (numeric rating scale, Tampa scale for kinesiophobia-11, Oswestry disability index and the Roland-Morris questionnaire, respectively). Results: The results indicate that both groups improved in the measured outcomes, with the only difference between them being a better improvement in kinesiophobia in the group receiving physiotherapy and pain neuroscience education. Conclusions: The results of this study confirm that a relatively short intervention of pain neuroscience education enhances the effects of physiotherapy and should be implemented in clinical practice.

Title: Salt Consumption, Obesity and Hypertension in Young Adult Girls in Mumbai
In Indian adolescents and young adults, prevalence of hypertension is increasing. Increased prevalence is because of many factors like change in eating habits, consumption of processed food, food junk and sedentary lifestyle. Hypertension which develops in childhood/ adolescence persists in adulthood. Therefore this study was conducted to determine salt consumption, obesity and hypertension in girls between 18-20 years of age. The study included 120 girls from different colleges in Mumbai. Data was collected using a questionnaire. Height, weight was measured and BMI was calculated. Blood pressure was measured using a digital sphygmomanometer. Seventy-nine girls had normal blood pressure whereas 41 girls were hypertensive. In the hypertensive group 11 girls had elevated BP, 18 girls had stage I HT and 12 girls had stage II hypertension. There was a significant correlation between BMI and HT. In the study 36.7% girls were underweight and 23.3% girls were overweight 4.5% girls were obese. The highest percentage of underweight girls was observed in stage II hypertension (41.7%). Girls were found to be consuming a lot of salty foods (pickle, pizza, burger, cheese, mayonnaise, Indian fried snacks- chakali, kachori, salted boondi, salted peanuts, popcorn, cheeselings, French fries, soya chips, corn chips, wheat puffs etc). No significant correlation was observed between sodium intake and BP. However, the highest mean consumption of salty foods was found in stage I HT subjects compared to elevated and stage II HT subjects. Sedentary activity was reported by 92% subjects. Only 6.7% had moderate activity and very few (0.8%) were doing heavy physical activity. The present study suggests that healthy diets and physical activity are important to reduce the risk of hypertension in young girls.

Title: Strategies to Alleviate the Burden Experienced by Informal Caregivers of Persons With Severe Mental Disorders in Low- and Middle-Income Countries: Scoping Review
There is considerable evidence of the burden of care encountered by informal caregivers of persons with severe mental disorders in low- and middle-income countries. This scoping review aims to identify and describe the extent and type of evidence on the existing strategies for alleviating the burden of care among informal caregivers of persons with severe mental disorders in low- and middle income countries. A systematic literature search was completed following the Joanna Briggs Institute methodology for scoping reviews. The participants, concept, and context framework was used to guide the search for literature sources across 5 databases: PubMed, MEDLINE, CINAHL, and PsycINFO for published literature and ProQuest for unpublished literature. This review included studies that reported on evaluated or recommended caregiver interventions and support strategies in low- and middle-income countries. The search was limited to studies conducted between 2001 and 2021, and only papers written in English were considered for inclusion. Using the Covidence software, 2 reviewers independently screened the papers, applied the inclusion and exclusion criteria. The relevant studies and reported outcomes were summarized, organized, and analyzed descriptively using numeric summary analysis and deductive content analysis. Of the 18,342 studies identified, 44 (0.24%) met the inclusion criteria. The included studies were from 16 low- and middle income countries in Asia, Africa, Europe, and South and North America. The identified strategies were grouped into 2 categories: implemented and recommended intervention strategies. Identified strategies included community-based interventions, psychoeducation interventions, support groups, cognitive behavioral therapy, spirituality-based interventions, smartphone-based interventions. and In addition, mindfulness and empowerment, collaborative interventions, standard care, financial and social support, counseling, occupation-based interventions, policy and legislature, and access to mental health care were identified. The variables in this research consist of independent variables namely maternal age, maternal occupation, maternal knowledge, family support, support from health workers, and the IMD process. The dependent variable is exclusive breastfeeding. This research obtained 71 samples of breastfeeding mothers. The research results showed that the majority respondents (78.9%) who gave exclusive breastfeeding were aged 25-35 years, had a high level of good knowledge and not work. All respondents also received support from families and health workers, as well as the majority of respondents (85.9) who breastfed Exclusively undergoes the IMD process. Secilia Serly Kebo, born in Larantuka 07 November 1982. In 2006 she graduated as a midwife and worked as a midwife in a very remote village. There were many shortcomings experienced while being a village midwife, namely electricity problems, internet networks and difficulties with public transportation. In 2009 she moved to work. a Community Health Center which is also still in a remote area. Working as a midwife at a Community Health Center, I experienced many things. I had to understand the culture and traditions of the community which presented many challenges in serving maternal and child health. In 2019 I continued my midwife education at Airlangga University, Surabaya and interested in continuing to serve the community, especially mothers and babies, with an approach to families and society. The approach taken is expected to support the health of mothers and babies. Many things that exist in society can be used as learning material to see the factors that influence each other on the development of maternal and child health. To achieve a good level of maternal and child health we need many factors that mutually support each other. I believe that by understanding all the supporting factors that exist we are able to create improvements and improve the status of public health.
Title: APOROPHOBIA: a useful and current reflection for tomorrow’s social complexity
The contributions presented address aporophobia in contemporary Brazilian society, focusing on various dimensions of the phenomenon. It highlights the contradiction between ethical principles, proclaimed human rights and the practice of systemic rejection of poverty. Aporophobia is associated with hate crimes against the poor, highlighting a conflict between the protection of individual rights and the fight against discrimination. The relationship between economic inequality, structural poverty and aporophobia is highlighted, emphasizing the importance of disseminating the debate in educational and media contexts. The urgency of tackling social inequalities is recognized, especially in the face of the necropolitics exacerbated by the COVID-19 pandemic. The influence of political discourses on the culture of hate is discussed, pointing to the need for measures that promote social inclusion and the eradication of poverty. In the context of the fight against aporophobia, the importance of effective public policies aimed at social equity and need for measures that promote social guaranteeing basic human rights is highlighted. In summary, the collective contributions converge towards a critical reflection on building a fairer, more inclusive and humane society, pointing out ways to prevent and eliminate aporophobia.

Title: Quality assessment of diabetic care in Armed Force Hospital Southern region
Objective: This study aimed to assess the current status of care provided by the Diabetes Center at Armed Forces Hospital (AFH).
Methods: This study was cross-sectional and conducted at the AFH in the Southern region during the period from January to June 2023. The study was conducted using the patient’s data sheet and a questionnaire to investigate the demographics and clinical characteristics of patients as well as the quality of care provided to the patients.
Results: This study included 180 diabetic patients; 52.2% were males, 61.1% had uncontrolled blood sugar, 37.8% were hypertensive, and 45% were obese. Around 60.6% reported receiving high-quality care. The quality of care was significantly affected by secondary education (p = 0.037), university education (p = 0.006), monthly income of more than 10,000 SR (p = 0.022), glycemic control (p = 0.039), and body mass index (p = 0.0001).
Conclusion: A good level of quality care was provided to the diabetic patients; however, it requires more improvement.

Title: Case of treatment of solid pseudopapillary pancreatic tumor
Solid pseudopapillary tumor of the pancreas, accounting for less than 3% of all pancreatic neoplasms, predominantly affects young women between the ages of 20 and 30. The prognosis for this condition is generally optimistic, as more than 85% of patients experience a complete cure following successful tumor resection. A clinical case from this publication illustrates the outcomes of surgical intervention in a patient with a malignant pancreatic tumor. The patient, known as M., born in 1998, received inpatient treatment at the prestigious I.K. Akhunbaeva National Hospital of the Ministry of Health of the Kyrgyz Republic from January 12, 2015, to January 26, 2015, presenting with a pancreatic head lesion discovered during surgery. Consequently, the decision was made to extend the surgical procedure. Subsequent histopathological examination confirmed the presence of a solid pseudopapillary tumor. Further immunohistochemical analysis identified positive expressions of synaptophysin, vimentin, neuron-specific enolase, B-catenin (nuclear reaction), PR, CK18, and CD10 in the tumor cells, assisting in characterizing the lesion histologically. This rare tumor type typically remains asymptomatic and is often incidentally detected or discovered upon reaching a significant size. Remarkably, the patient in this case displayed a favorable clinical progression without any indicators of disease advancement or recurrence over the seven-year post-treatment follow-up period. In conclusion, solid pseudopapillary tumor signifies a rare yet highly specialized malignant growth that predominantly emerges in young females. The clinical narrative depicted in this case study underscores the generally positive nature of this condition.

Title: Functional Outcome of Subvastus versus Medial Parapatellar Approaches for Total Knee Replacement in Patients with Knee Osteoarthritis: A Prospective Cohort Study
Background: Subvastus approach and medial parapatellar approach are two major approaches for total knee replacement (TKR). There is no global consensus on the superiority of either approach in terms of functional outcomes. Objective: The present study aimed to evaluate the functional outcome of TKR through subvastus approach and medial parapatellar approach by using patient-reported scores at 3-, 6-, and 12-month post-operative follow-ups.
Methods: This prospective cohort follow-up study included patients with knee osteoarthritis who underwent elective primary TKR either through the subvastus or medial parapatellar approaches at King Abdullah Medical City, Makkah city, Kingdom of Saudi Arabia, from January 2019 to December 2022. Scores from the self-reported Oxford Knee Score (OKS) and the Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) were compared in the two groups of patients at 3-, 6-, and 12-month post-operative follow-ups.
Results: A total of 98 patients were included, of which 37 underwent TKR through the subvastus approach and 61 through the medial parapatellar approach. There was an overall significant change over time in both WOMAC and OKS scores (P < 0.001). Patients who underwent the subvastus approach had significantly higher mean of WOMAC and OKS than patients with the medial parapatellar approach at the 3- and 6-month follow-ups (P < 0.05), but not at the 12-month follow-up.
Conclusions: For TKR, the medial parapatellar approach results in better functional outcomes at the 3- and 6- month follow-up periods compared with the subvastus approach.

Title: Resilience strengths and processes in families with adolescent children: A contribution from the nurse specialist
Adolescence brings along new challenges, with effects on the families daily lives, which change their dynamics and lead to constant challenges and consequent adjustments in interfamily relationships. The aim of this study is to analyze the characteristics of parents and adolescents that are related to the strengths and processes of resilience in families with adolescent children. This is a quantitative, descriptive and correlational study whose data collection was carried out online through the application of a sociodemographic questionnaire, the Family Strengths Questionnaire (Melo & Alarcão, 2011) and the Communication Assessment Scale in Parenting (COMPA- P) (Portugal & Alberto, 2014). The sample consists of parents with teenage children between 14 and 17 years old. It suggested that family sociodemographic variables were not associated with family resilience processes. Regarding parental communication, there was a positive and significant relationship between its dimensions and the dimensions of the Family Strengths questionnaire. As for the characteristics of the adolescent, it was concluded that the health status was positively associated with family resilience processes. The regression model demonstrates that the adolescent’s health status, as well as the dimensions of meta-communication and trust/sharing of parents towards their children, are explanatory factors of the strengths and processes of family resilience. If nurses know the resilience forces and processes of families, they will be able to more easily develop adapted and appropriate responses to each family in order to promote family resilience, as it has a strong effect on the health and well-being of family members and on the family itself as a system.
